Please complete all of the information requested below. Remember that use of the amenities is not final until the office provides you with written confirmation.
Receipt of security deposit and rental payments are required before reservation confirmation will be issued.
Mail two checks or money orders (no cash) - one for the refundable security deposit and one for the rental fee (Rental Fee + Additional Hours + Cleaning Fee) to:
